I hereby give permission to Jenna Elizabeth Studios to use me and or my minor(s) photo(s) and likeness in all forms and media for advertising, portfolio, demo, trade, stock photography, editorial, altering without restrictions, and all other lawful purposes. I understand I am entitled to no further compensation other than what has been outlined herein this contract. I release the photographer of all forms of claims and liability related to my photo usage.
I understand all portraits will be used to promote Jenna Elizabeth Studios LLC, models do NOT have the right to sell, share, or enter portraits into contest or talent agencies without prior written authorization from the studio.
Model compensation is strictly in the form of one free 8 x 10 matted folio print (valued at $250.00) and does NOT include any additional portraits, digital negatives, studio credits, cash payment, or any other compensation. Models wishing to return and view portraits may select the gifted portrait. Otherwise, Jenna Elizabeth Studios will select the portrait on behalf of the model.
Compensation is made once and only in the case that all appointments are honored as originally scheduled. In the case of a canceled or rescheduled appointments, compensation is forfeited and fees may apply to reschedule.
Upon submitting this questionnaire, prospective participants agree to receive further communications for Jenna Elizabeth Studios. We do not sell or share your information with outside parties.
